Failed trials in acute mania, however, as well as the drug's failure to prevent manic relapse in maintenance studies, indicate that lamotrigine cannot be considered a bidirectional mood stabilizer. Ultimately, lamotrigine may become more important in the long-term management of patients with bipolar II disorder, in which depression is a more serious threat than hypomania. The drug may also serve as an adjunct treatment for maintenance in patients with bipolar I disorder. Lamotrigine carries a black box warning for life-threatening rash, which necessitates a gradual titration of the drug to therapeutic doses to limit this risk. This slow titration severely limits the agent's potential use in patients who are acutely depressed. In addition to the risk of rash, lamotrigine has been associated with headache, nausea, and dizziness. Other anticonvulsants, such as gabapentin and topiramate, have not demonstrated significant utility as monotherapy in controlled trials in patients with bipolar disorder. Interest continues, however, in the usefulness of topiramate as an adjunct treatment for the disorder and monoket and gabapentin.
A particular concern with the current pharmaceutical industry practice is the process of `evergreening, ' where manufacturers of brand name drugs make variations to existing drugs in order to extend their patent coverage. This delays the ability of generic manufacturers to develop cheaper products for the marketplace and it is a questionable outcome of Canada's patent law. Furthermore, regulations under the patent law require generic drug manufacturers to demonstrate that their product is not infringing on a patent held by another drug manufacturer rather than putting the onus on the patent drug manufacturer to show that their patent has been infringed what is referred to as the notice of compliance regulations. Suggestions have been made that this leads to "pre-emptory" lawsuits from patented drug manufacturers as a way of delaying the approval of generic drugs. Clearly, if this is the case, the practice is not in the public interest. The federal government should review this issue, determine what constitutes a legitimate extension of patent protection, and also consider ways of streamlining the approval of generic drugs.
Patients with Lennox-Gastaut syndrome often experience numerous seizures per day, some of which are absence seizures that may be difficult to identify and monitor. Consequently, tonic or atonic seizures are generally the only types of seizures considered in study outcomes in this patient population. The guideline panel determined that lamotrigine, in weight-based doses, is effective for the treatment of tonic and atonic seizures caused by Lennox-Gastaut syndrome in adults and children Level A 45, 46 additional data led to a recommendation that topiramate is effective for the same indication Level A ; .47 No Class I or Class II evidence was available to support the use of gabapentin, tiagabine, oxcarbazepine, levetiracetam, or zonisamide for the treatment of seizures associated with Lennox-Gastaut syndrome Level U ; . Notably, case reports have indicated that both lamotrigine and gabapentin may worsen the incidence of myoclonic seizures, in which limbs jerk suddenly, in this patient population.48 and imdur.
